Job summary

Amerita is seeking a Pharmacist to join the High Point, NC pharmacy team to manage home infusion patient care and drug therapy planning. You will collaborate with physicians, nurses and distribution to ensure safe medication use and effective therapy in the home setting.

The ideal candidate holds a PharmD or BS in Pharmacy with an active license and 2+ years of acute-care or home infusion experience. A professional development culture and clear career progression discussions are offered.

Qualifications

• Bachelor’s degree in Pharmacy required; PharmD preferred.
• Registered Pharmacist license in state(s) of practice.
• Minimum of two years of recent experience in an acute-care setting or one year in home infusion therapy.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Good organizational and time-management skills.
• Excellent verbal and written communication skills.

Responsibilities

• Evaluate and monitor drug therapy for home care patients in collaboration with physicians, nurses and other healthcare professionals.
• Adhere to all applicable laws, regulations and accreditation standards governing pharmacy practice.
• Coordinate and facilitate new referrals and admissions with nurses, customer service and distribution.
• Review and verify all new patient orders and document on the order form.
• Assist physicians and other professionals with home care patient planning and monitoring of drug therapy.
• Conduct ongoing assessment of patient, drug and disease information to identify drug-related problems.
• Maintain thorough documentation in patient records of all clinical activities and communications.
• Process and check medications, supplies and equipment prior to dispensing.
