Job summary

Option Care Health is seeking an Infusion Nurse II to deliver intermediate nursing care at patients' homes or alternate infusion suites. You will follow a physician plan of care, ensuring safety, continuity, and timely documentation while coordinating with clinical teams.

Requires an active RN license, 1 year of nursing experience, and CPR certification. In a nationwide home infusion network, you’ll contribute to a compassionate and compliant care model with opportunities for professional growth.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers.
• Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals.
• Assesses patient needs and physical status at each skilled visit through health data access and patient interview.
• Coordinating discharge planning and prepares discharge summaries with patient instructions and progress notes.
