Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or alternate infusion sites. You will work under supervision to ensure patient safety and adherence to the plan of care.

Active RN license in the state, 1+ year nursing experience, and CPR certification are required. Travel to patient sites may be involved, with a focus on delivering high-quality infusion services nationwide.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 years of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and implements intermediate nursing plan of care in home settings.
• Assesses patient needs and updates plan per physician orders.
• Provides training and mentorship to Infusion Nurse I’s.
• Communicates with clinical team and family to ensure continuity of care.
• Documents care in real-time and supports billing processes.
• Coordinates discharge planning and patient education.
• Ensures compliance with policies, procedures, and regulations.
• Mentors nurses and participates in team conferences.
