Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care at home or in an infusion suite. You’ll work under supervision to ensure patient safety and continuity of care under a physician plan of care.

The role requires an active RN license in the state, 1+ year of nursing experience, and CPR certification. Work involves direct patient care, documentation, and collaboration with a multidisciplinary team in a home setting with potential travel.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required (an in-person class is required per company policy).

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments and educates patients and families/caregivers.
• Assesses patient needs and physical status at each skilled visit and communicates with physician orders.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Provides effective patient teaching to achieve independence with prescribed therapy.
• Communicates with clinical team, documents care in real time for billing and continuity of care.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
