Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate patient care in the home or alternate infusion suite under a physician’s plan of care. Works under structured supervision to ensure safety and continuity of care.

You will assess and treat patients, educate families, document in real time, and collaborate with the clinical team. Requires RN license, CPR, and 1 year nursing experience; travel up to 50% is expected.

Qualifications

• Active and unrestricted RN license in the practice state.
• Minimum 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and provides intermediate direct patient care in home or infusion suite under physician plan.
• Assesses patient needs, documents care in real time, and communicates with care team.
• Educates patients and families/caregivers for therapy and home care goals.
• Coordinates discharge planning and follow-up with multidisciplinary team.
• Performs other duties as assigned while maintaining compliance with HIPAA and policies.
