Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite. You will ensure patient safety under a physician’s plan of care and work under supervision.

The role requires an active RN license, CPR, and at least 1 year of nursing experience. Will travel to patient homes as needed, with training and clinical support provided.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments.
• Assesses patient needs and status during visits and updates plans accordingly.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Educates patients and families to achieve independence with prescribed therapy.
• Documents care in real-time and coordinates with clinical teams for continuity of care.
• Coordinates discharge planning and provides clear discharge instructions.
