Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the patient's home or infusion suite, under physician orders. This licensed RN role emphasizes patient safety, documentation, and collaboration with the clinical team.

The position requires an active RN license, at least 1 year of nursing experience, and CPR certification, with responsibilities including plan-of-care implementation, patient education, and discharge planning in a home

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit; reevaluates and updates plans as needed.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Communicates with the Clinical team, medical providers, patients, and families to ensure continuity of care.
• Prepares clinical documentation in real time during visits and submits to the nursing department for timely billing and records.
