Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite under a physician's plan of care. The role operates under the supervision of a designated supervisor and emphasizes safety, compliance, and patient education.

The position requires an active RN license in the practice state and CPR certification, with at least 1 year of nursing experience.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Current CPR certification required (in-person class as per policy).
• Minimum of 1 year nursing experience.

Responsibilities

• Initiates and adjusts the nursing plan of care for home/in‑suite patients, administers medications, and educates patients and families.
• Assesses patient needs at each visit, documents outcomes, and communicates significant changes to the physician.
• Provides training/mentorship to Infusion Nurse I to ensure patient safety and compliance.
• Communicates with clinical teams, physicians, patients, and families to coordinate care and comply with timelines.
• Coordinates discharge planning and prepares discharge instructions and patient progress reports.
