Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite, ensuring safety under a physician's plan of care. You will collaborate with the clinical team and educate patients and families while documenting care in real time.

The role requires an active RN license, at least 1 year of nursing experience, and CPR certification, with responsibilities including discharge planning and mentoring junior nurses.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and provides intermediate nursing care and educates patients and families.
• Documents care in real time and communicates with clinical team.
• Coordinates discharge planning and reports progress to physicians.
• Mentors Infusion Nurse I staff to ensure patient safety.
• Adheres to HIPAA and agency policies during home visits.
