Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or alternate infusion suite under a physician's plan of care. The nurse works under structured supervision to ensure patient safety and continuity of care.

The role requires an active RN license, at least 1 year of nursing experience, and CPR certification. It is a full-time position in Brooklyn/NY with opportunities to grow within a large national infusion provider.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and implements intermediate nursing plan of care treatments and educates patients and families.
• Assesses patient needs during visits and communicates significant changes to physicians.
• Provides training to Infusion Nurse I staff and ensures patient safety and compliance.
• Maintains real-time clinical documentation for billing and continuity of care.
• Coordinates discharge planning and prepares discharge summaries with clear instructions.
