Option Care Health is seeking an Infusion Nurse II to coordinate and deliver intermediate direct patient care in the home or in an alternate infusion suite, ensuring patient safety under a physician's plan of care.

Under structured supervision, the nurse initiates and adjusts nursing plans, educates patients and families, and collaborates with the clinical team to maintain continuity of care and compliance with HIPAA and agency policies.

Formación

• Licensed registered nurse with active licensure.
• Experience coordinating home infusion care preferred.

Responsabilidades

• Coordinate and provide intermediate direct patient care in home or infusion suite.
• Initiate, develop, and implement nursing plan of care and educate patients and families.
• Assess patient needs at each visit and adjust plan per physician orders.
• Provide training to Infusion Nurse I's to ensure safety and compliance.
• Document care in real-time and communicate with the clinical team.
