Cityscape Healthcare is seeking experienced Nurse Practitioners or Physician Assistants to join our growing mobile care network in Norfolk, Nebraska. This role offers the opportunity to provide in-home and in-clinic care, perform health assessments, manage chronic and acute conditions, and deliver advanced wound care.

Providing a high level of autonomy, you will collaborate with a supportive clinical team. Candidates must possess a valid NP or PA license and experience in wound care or home-based care. Reliable transportation is required for travel to various locations.

Formación

• Active and unrestricted NP or PA license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation and valid driver’s license.

Responsabilidades

• Deliver advanced wound care services including assessment and treatment planning.
• Manage chronic and acute conditions using a comprehensive approach.
• Document patient encounters accurately within the EMR system.
• Collaborate with care teams to ensure continuity of care.
