Job summary

QuantumCare is seeking motivated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our expanding mobile wound care team. You will provide in-home and in-clinic visits, perform health assessments, and deliver advanced wound care in Nebraska and surrounding areas.

You will manage chronic and acute conditions with a whole-patient approach, document encounters in our EMR, and coordinate with care teams to optimize outcomes while complying with Medicare and state regulations.

Qualifications

• Active NP or PA license in NE or IA.
• Experience in wound care, home-based care or geriatric medicine preferred.
• Ability to travel day trips and work independently.

Responsibilities

• Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
• Deliver mobile wound care services directly in patients' homes or care facilities, ensuring comfort, accessibility, and continuity of care.
• Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
• Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
• Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
• Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
• Other duties as assigned.
