Job summary

Qualifications

• Active and unrestricted NP or PA license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide advanced wound care treatment for patients with complex wounds, including assessment, debridement, and treatment plans.
• Deliver mobile wound care services in patients' homes or care facilities, ensuring comfort and continuity of care.
• Diagnose and manage chronic and acute conditions using a whole-patient approach.
• Document all patient encounters and treatments in the EMR per organizational standards.
• Coordinate with internal and external teams to optimize outcomes.
• Ensure compliance with Medicare, state regulations, and protocols.
• Other duties as assigned.
