Position Overview
We are seeking motivatedNurse Practitioners (NPs)orPhysician Assistants (PAs)to join our expanding mobile care team. In this role, you will provide in-home and in-clinic visits, perform health assessments, manage chronic and acute conditions, and deliver advanced wound care. This is an opportunity to practice meaningful, relationship-based medicine while working autonomously in a flexible, supportive environment.

Services:

• Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
• Deliver mobile wound care services directly in patients’ homes or care facilities, ensuring comfort, accessibility, and continuity of care.
• Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
• Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
• Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
• Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
• Other duties as assigned.

Qualifications:

• Active and unrestricted Nurse Practitioner or Physician Assistant license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation and valid driver’s license. The position involves day-trip travel to patient homes and partner facilities.
• Strong clinical assessment, communication, and documentation skills.
• Self-starter comfortable working independently with mobile technology.
