Job summary

West Coast Wound & Skin Care is seeking a qualified Physical Therapist to deliver in-home services across the Hemet/Temecula region. You will perform evaluations, create individualized plans of care, and provide interventions to improve mobility and safety in the home setting.

The role is a 1099 independent contractor position requiring California PT licensure, strong clinical judgment, and excellent communication with patients and caregivers. Travel to patient homes is required.

Qualifications

• Current and valid California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
• Excellent patient assessment and communication skills.

Responsibilities

• Travel to patients' homes within Hemet/Temecula region to provide physical therapy services.
• Perform comprehensive home-based PT evaluations and develop individualized plans of care.
• Provide skilled interventions and monitor progress, adjusting plans as needed.
• Educate patients and caregivers on exercises, mobility, safety, and fall prevention.
• Maintain accurate documentation and ensure compliance with regulatory requirements.
