Job summary

Catholic Health Initiatives seeks a Job Coach to empower people with developmental disabilities to live more independently and participate in community life. You will support goal achievement, health and wellness, and advocate for person-centered plans in daily activities.

You will learn about each person’s goals, coordinate with teams, and help maintain natural supports while ensuring safety and quality of life across all settings.

Qualifications

• High School diploma or equivalent is required.
• Driver's license must be valid at time of hire (ND).
• Auto insurance is required at hire.

Responsibilities

• Ensures each person has all basic assurances met.
• Attends program training with the QIDP and implements OSP/PCSP programs and supports.
• Documents accurately in all records and files for people supported.
