Job summary

ADAPT Community Network is seeking a Community Habilitation Specialist to deliver individualized training and personal assistance to participants with developmental and/or physical disabilities in homes and communities.

You will promote meaningful independence, personal growth, and full community inclusion while working with minimal supervision and collaborating with a cross-functional team.

Qualifications

• High School Diploma or equivalent required.
• Some college coursework in human services preferred.
• Experience working with individuals with developmental or physical disabilities preferred.
• Ability to read, write, speak, and understand English.
• Strong interpersonal and documentation skills.
• Willingness to travel to participant homes and community locations.

Responsibilities

• Provide habilitation supports per each participant’s plan.
• Assist participants in identifying goals, preferences, strengths, and learning needs.
• Support participation in meaningful activities reflecting person-centered practices.
• Promote independence through structured routines and prompting strategies.
• Maintain confidentiality and complete accurate session notes and progress summaries.
