Job summary

Northpointe Behavioral Healthcare System is seeking a Community Support Specialist to coach and assist individuals in achieving independence and community inclusion. You will collaborate with clinical staff to implement plans and support daily living activities in home and community settings.

Responsibilities include documenting progress, transporting clients safely, participating in team meetings, and applying person-centered strategies to promote safety and well-being.

Qualifications

• High school diploma or GED is required.
• Experience with individuals with developmental and/or intellectual disabilities preferred.
• Experience in caregiving role preferred.
• Valid driver’s license preferred.
• Certification as a nursing assistant preferred (re-certification required for compensation).

Responsibilities

• Assists clinical staff in administering and documenting therapeutic activities.
• Safely transports and accompanies individuals to and from community needs according to the plan.
• Attends and participates in staff meetings; protects Recipient Rights.
• Observes and documents behaviors to support treatment goals.
• Provides verbal, nonverbal, or physical interventions as trained.
• Interacts with individuals in daily activities and adapts tasks as needed.
• Prompts and cues bathing, hygiene, and dressing according to the plan of service.
• Records data into the individual’s electronic medical record.
• Maintains a safe and therapeutic environment for individuals and staff.
• Provides prompts for housekeeping to maintain safety and cleanliness.
• Maintains a safe driving record.
