Job summary

Trinity Services, Inc. seeks an Independent Living Coach to support adults with intellectual disabilities and/or mental illness in their homes and community programs. You will coach, guide, and empower individuals to learn life skills and increase independence, providing empathy, patience, and person-centered support.

No prior field experience is required; training is provided. You’ll work with a Team Leader/Program Coordinator, offering transportation, daily living assistance, and participation

Qualifications

• A valid High School Diploma or GED is required.
• A valid driver’s license with clean driving record.
• Pass a STOFLA literacy assessment before start date.
• CPR/First Aid, Safety Care certifications and Medication Administration Authorization required.

Responsibilities

• Foster meaningful relationships with people receiving services.
• Provide transportation in an agency vehicle to appointments and community activities.
• Assist with daily living activities, meal prep, and planning meaningful activities.
• Document activities and services provided to individuals.
• Monitor medical/behavioral changes and report to nurse and supervisor.
