Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator / Case Manager to join our team. You will develop individualized care plans and coordinate services to help adults live independently in their homes and communities.

The role involves monitoring outcomes, ensuring access to housing, medical, social, and community supports, and maintaining confidentiality in all interactions. Travel up to 50% may be required.

Qualifications

• BS/BA degree in related field or equivalent.
• Minimum 1 year of case management experience with IDD, autism, or physical disabilities.
• Experience with Microsoft Office and state databases.
• Maintain CPR/First Aid certification and required state clearances.
• Valid driver’s license and personal vehicle insurance; vehicle registration must be current.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update at least annually.
• Ensure participant has informed choice of providers.
• Ensure ISP alignment with Behavioral and Crisis Plans (if applicable).
• Coordinate with care teammates to ensure seamless delivery of services.
• Research and implement alternative solutions to meet participant needs.
• Maintain confidentiality of sensitive information.
• Attend trainings and recertifications; communicate updates to supervisor.
• Travel to participants up to 50% as needed.
