Care Lync in Maine is seeking a dedicated Support Coordinator to empower adults and aging individuals to live independently. You will assess needs, develop Individual Support Plans, arrange services, and coordinate with medical professionals and community resources.

Travel up to 50% to meet participants in homes and communities, maintain documentation, and participate in ongoing training and career growth within a collaborative, inclusive team.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• 3+ years of experience providing case management or support coordination to individuals with disabilities and/or aging adults.
• Strong organizational, communication, and advocacy skills.
• Comfort traveling up to 50% to meet with participants in homes and communities.

Responsabilités

• Conduct person-centered assessments to identify participant needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISP) to ensure appropriate services.
• Provide participants with choice and advocacy in selecting providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver high-quality care.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
