Strong organizational, communication, and advocacy skills.
Willingness to travel up to 50% to meet participants.
Valid driver's license, auto insurance, and reliable transportation.
Commitment to maintaining required certifications and state clearances.

Responsibilities

Conduct participant-centered assessments to identify needs, preferences, and goals.
Develop, implement, and update Individual Support Plans (ISP).
Provide participants with choice and advocacy in care options.
Coordinate with medical professionals, behavioral specialists, families, and community resources.
Monitor services for safety, satisfaction, and progress toward independence.
Support adults with disabilities and aging population in accessing community resources.
Maintain documentation, confidentiality, and program compliance.
Participate in ongoing training and professional development.
