Qualifications

• Provide case management for clients as assigned and support daily living skills.
• Coordinate services across medical, psychiatric, dental, and social supports.
• Document progress and maintain service plans and records.

Responsibilities

• Provide case management for residents to attain daily living skills.
• Act as liaison for services per the comprehensive service plan.
• Coordinate care with Beacon of Hope and other providers.
• Make home visits and document progress within 24 hours.
• Ensure safety and report concerns per program standards.
• Assist with maintenance tasks and arrange collateral contacts.
• Escort clients to appointments and provide crisis intervention when needed.
• Comply with NY OMH regulations and agency policies.
