Catholic Charities of New York is seeking an Apartment Program Case Manager in the Bronx to assist residents with community life skills per their service plans. This full-time, non-exempt role reports to the Program Director and requires coordinating with multiple service providers and agencies.

The position involves home visits, safety assessments, crisis intervention, and documentation within the AWARDS system, with duties extending to after-hours coverage as needed.

Qualifications

• Bachelor’s Degree with two years of Case Management experience.
• High School Diploma with four years of Case Management experience.
• Experience serving individuals with serious mental illness preferred.

Responsabilités

• Provide case management for clients; help residents acquire daily living skills.
• Liaise for services: scheduling, referrals, follow-up; coordinate medical and social needs.
• Integrate resident care with staff and external providers; document progress in AWARDS.
• Make regular home visits; assess safety and report concerns to the Program Manager.
• Assist with maintenance tasks and ensure compliance with program standards.
• Maintain collateral contacts; escort clients to appointments as needed.
• Provide crisis intervention with NYPD/EMS support when required.
• Adhere to NY State OMH regulations and CCCS/BOH policies; participate in required trainings.
