Job summary

Institute for Community Living Inc. in Brooklyn, NY, seeks a dedicated case manager to support individuals in independent community living settings, coordinating medical, psychosocial, educational and vocational services.

You will conduct assessments, develop service plans, and provide crisis intervention as part of a multidisciplinary team, traveling to consumer residences as needed.

Qualifications

• Education: Bachelor's degree plus two years’ experience providing direct services to individuals with mental disabilities or related human service experience, or a related practicum.
• Ability to work with consumers/residents, families, and staff in a caring and respectful manner.
• Willingness to participate in in-service training and development.

Responsibilities

• Travel to consumers' residences to provide supportive services.
• Maintain consumer treatment plans and quarterly reviews.
• Maintain up-to-date case records and document progress.
• Provide individual or group counseling to modify attitudes and behaviors.
• Disseminate information to consumers in a clear manner.
• Use cognitive-behavioral techniques to assist in meeting service plans.
• Assist in discharge planning and link to community resources.
• Advocate for consumers with external service providers.
