Job summary

Catholic Charities of New York's Apartment Program is seeking an Apartment Program Case Manager based in Bronx, NY to help residents learn life skills and follow a comprehensive service plan. Location is 2510 Westchester Avenue, Bronx, NY, with salary $27.22–$28.90 per hour.

The role requires frequent home visits, collaboration with medical and social services, and crisis intervention support. Strong documentation and travel across boroughs are essential for success.

Qualifications

• Experience coordinating services for individuals in need.
• Ability to link residents with medical, psychiatric, dental and entitlement services.
• Strong communication and documentation skills.

Responsibilities

• Provide case management for clients as assigned and help residents attain daily living skills.
• Act as liaison for services needed by residents per their service plan.
• Integrate care with Beacon of Hope staff and other providers.
• Make multiple visits per month to assigned clients at home or other locations.
• Perform ongoing environmental assessments of apartments for safety and compliance.
• Assist with general maintenance tasks as needed.
• Establish and document collateral contacts with residents’ contacts and providers.
• Escort clients to appointments including medical and social services.
• Provide crisis intervention with NYPD, Mobile Crisis, and/or EMS.
• Use AWARDS system to input progress notes within 24 hours and complete service plans.
• Attend meetings, trainings, and conferences as required.
• Adhere to NYS regulations for housing programs and CCCS/BOH policies.
• Perform other duties as assigned by Program Manager.
