ICL is seeking a dedicated community support worker to assist individuals living in independent apartments within the New York area. You will provide counseling, assess needs, and link residents to medical, social, educational and vocational resources to promote independent living.

Travel to consumer residences, develop service plans, monitor progress, and document outcomes while collaborating with the team.

Qualificações

• Bachelor's degree required or equivalent experience in human services.
• Direct service experience with mental disabilities or related fields preferred.
• Ability to document and maintain case records in accordance with policy.

Responsabilidades

• Provide supportive counseling to individuals and groups.
• Travel to consumer residences and community sites to assist with services.
• Develop and monitor service plans and track progress.
• Coordinate with multidisciplinary team and community resources.
• Ensure safety, confidentiality, and regulatory compliance in all activities.
