The role requires a Bachelor's or equivalent CM experience, state driving optional, and a commitment to safety, documentation, and crisis intervention when needed.

Qualifications

• Education/experience aligned with mental health housing programs.
• Ability to interpret service plans and coordinate care.
• Strong written and verbal communication skills.

Responsibilities

• Provide case management to residents to achieve daily living skills.
• Liaise with services: medical, dental, entitlements, social programs.
• Visit clients in homes and other locations to monitor progress.
