Qualifications

• Bachelor's degree or higher in relevant field; two years’ direct service experience preferred.
• Experience providing mental health/substance use supports and crisis intervention.
• Ability to document progress, develop treatment plans, and maintain case records.

Responsibilities

• Travel to consumer residences to provide supportive services and counseling.
• Develop and monitor consumer treatment plans and progress notes.
• Maintain up-to-date case records and treatment progress documentation.
• Provide crisis intervention and assist with discharge planning and community referrals.
• Advocate for consumers and coordinate with other service providers.
