Job summary

ICL is seeking a dedicated professional to support adults with mental health needs living in community-based settings in New York. You will travel to residences, assess needs, and help clients develop living, social, and vocational plans to maximize independence.

Responsibilities include creating treatment plans, providing individual and group counseling, coordinating with services, and ensuring progress through documentation and follow-up.

Qualifications

• Bachelor's degree plus two years’ direct service experience with individuals with mental disabilities or related human services.
• Ability to work with diverse consumers and maintain professional boundaries.
• Willingness to travel to consumers' residences and provide community-based support.

Responsibilities

• Travel to consumers' residences to provide supportive services and help develop plans for meeting service needs.
• Maintain consumer treatment plans, quarterly reviews, and records; document progress.
• Develop measurable service plans that consider cultural background and language.
• Provide 1:1 or group counseling to modify attitudes and behaviors as needed.
• Refer consumers to community resources and accompany them to appointments.
• Execute crisis assessment and nonverbal/verbal crisis intervention.
• Coordinate discharge planning and follow-up for continued community support.
