Job summary

ICL Bronx TAP 20Bed seeks a dedicated caseworker to support consumers living in independent apartments in the community. Under supervision, you help individuals develop living, working and social skills and connect them with medical, social and educational services.

You will travel to residences, maintain case records, implement service plans, provide crisis intervention and coordinate with a multidisciplinary team to promote recovery and independence.

Qualifications

• Bachelor's degree plus two years’ direct services experience, or practicum-based bachelor's degree with one year experience, or associate's degree with three years’ relevant background.
• Ability to work with individuals with mental disabilities in community settings and maintain accurate documentation.
• Willingness to participate in ongoing in-service training and adhere to regulations.

Responsibilities

• Travels to/visits consumers' residences to provide supportive services on a schedule set by the program.
• Maintains up-to-date case records and develops measurable service plans.
• Provides crisis intervention and collaborates with the team to promote independence.
• Assists consumers in developing living, working and social skills in the community.
• Explains and connects consumers with medical, social, educational, and vocational resources.
