FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver high-quality home health services in the Rocklin area as an Independent Contractor.

You will assess patients’ functional status, ADLs, and home safety, develop treatment plans, and lead interventions aimed at improving wellbeing. Collaboration with a care team and scheduling autonomy are key features of this role.

Qualifications

• Ability and enthusiasm in working with all ages.
• OT license and state registration required.
• Completion of an accredited Occupational Therapist program.

Responsabilités

• Assess patient care needs related to functioning, ADLs, and home safety.
• Develop and implement treatment plans; perform interventions to improve wellbeing.
• Evaluate patient progress and coordinate with health care team.
