Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier care and work with adults of all ages in the Sacramento area as an Independent Contractor.

The OT in Home Health assesses patient needs related to functioning, ADLs, fine motor, home safety, and adaptive equipment; develops treatment plan, delivers interventions, and monitors progress. A state OT license and a willingness to serve diverse populations are required; bilingual skills are a plus.

Qualifications

• Ability and enthusiasm in working with all ages.
• OT license and state registration.
• Completion of an accredited OT program.
• Bilingual is a plus.

Responsibilities

• Assess and evaluate patient functional status, ADLs, fine motor skills, home safety.
• Develop and implement an OT treatment plan.
• Perform interventions to improve wellbeing and function.
• Evaluate patient progress and adjust plan as needed.
• Educate patients, families, and caregivers on therapy activities.

FeldCare Connects, an app-based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Sacramento and the surroundingareas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
