Job summary

FeldCare Connects seeks a self-motivated Occupational Therapist to deliver premier care to adults in home health. This Independent Contractor role serves Sunland and surrounding areas, offering flexible scheduling and the autonomy to manage your own caseload.

Responsibilities include assessing functional status, ADLs, safety, and equipment needs; developing care plans; implementing therapy; and documenting progress while coordinating with the care team and educating patients and families.

Qualifications

• Must be licensed and registered as an Occupational Therapist in the state.
• Completed an accredited Occupational Therapist program.
• Bilingual is a plus.

Responsibilities

• Assess patient care needs related to function, ADLs, fine motor skills, home safety, and adaptive equipment.
• Develop and implement treatment plans to improve wellbeing and independence.
• Perform interventions and evaluate patient progress over time.
• Lead patients in therapy, self-care activities, and safety training.
• Observe, record, and report patient responses and changes in condition.
• Instruct patients, families, caregivers, and the health care team on therapy activities.

FeldCare Connects, an app-based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Sunland and the surroundingareas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
