Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier home health care to adults in Sunland and surrounding areas. This Independent Contractor role offers flexible scheduling and local assignments.

You will assess functional status, ADLs, home safety, and adaptive equipment needs; develop a treatment plan, perform interventions, and monitor progress to promote wellbeing and independence.

Qualifications

• Must be licensed Occupational Therapist with state registration.
• Completed an accredited OT program.
• Ability and enthusiasm to work with adults of all ages.
• Bilingual is a plus.

Responsibilities

• Assess and evaluate patient care needs related to functionality status, ADLs, home safety, and adaptive equipment.
• Determine and implement OT treatment plans and interventions to improve wellbeing.
• Evaluate progress and adjust plans as needed to optimize outcomes.
• Provide home assessments and recommendations for adaptive equipment.
