Purpose

Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier home health care for adults in the Hercules area. This Independent Contractor role focuses on functional assessment, ADLs, and safety in the home, with a plan of care tailored to each patient’s needs.

The OT will design and implement treatment plans, perform therapeutic interventions, and regularly evaluate progress, while engaging patients, families, and care teams to maximize independence and safety at home.

Qualifications

• Ability and enthusiasm in working with all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Bilingual is a plus!

Responsibilities

• Assess and evaluate patient needs related to functionality, ADLs, fine motor, home safety, and adaptive equipment.
• Develop and implement a treatment plan to improve wellbeing and progress.
• Perform interventions and evaluate patient progress over time.
• Educate patients, families, and care teams on therapy activities and participation.
