FeldCare Connects in Stockton, CA is seeking a self-motivated Occupational Therapist to provide home health services to adults of all ages as an independent contractor.

You will assess functional status, ADLs, and home safety, develop treatment plans, and coordinate with the health care team to support patient wellbeing.

Flexible scheduling, autonomy, and administrative support are offered, enabling clinicians to deliver high-quality care in a supportive work environment.

Qualifications

• Licensed OT with current state registration.
• Completion of an accredited OT program.
• Bilingual skills are a plus for patient and family communication.

Responsabilités

• Assess patient care needs related to functionality, ADLs, fine motor, and home safety.
• Develop and implement treatment plans, perform interventions, and evaluate progress.
• Provide OT services per state laws and plan of care in coordination with the health care team.
• Lead patients in therapeutic activities to improve functioning and safety.
• Observe, document and report responses; educate patients, families, and caregivers.
