Job summary

The Agency for Persons with Disabilities seeks an Incident Management Liaison to support incident reports, wellness visits, and APD investigations, ensuring timely entry into the IMS while upholding HIPAA requirements.

This professional role focuses on coordinating with regional teams and providers, reviewing daily data, and ensuring compliance with operating procedures to maintain safety and service quality across APD programs.

Qualifications

• Highschool Diploma or its equivalent.
• Experience using Microsoft Excel.

Responsibilities

• Forward critical incident reports to management staff and State Office based upon APD’s Incident Reporting Operating Procedure.
• Review incident reports submitted by providers and enter into the Agency’s Incident Management System (IMS).
• Follow up with providers as needed to obtain missing incident report information and ensure timely follow-up incident reports.
• Ensure wellness visits are conducted within required timeframes for allegations of abuse, neglect or exploitation.
• Review monthly DCF data downloads and ensure reporting of ANE findings; review ER/hospitalization data and ensure reports are submitted.
• Participate in meetings, conference calls, training and regional/quartely meetings with the QIO.
