Resources for Human Development is seeking an Investigator & Incident Coordinator to oversee incident management across our Pennsylvania programs. You will review reports, conduct thorough investigations, and ensure regulatory compliance while promoting quality improvement and safe services.

The role requires strong analytical, written and verbal communication skills, trauma-informed interviewing, and the ability to coordinate with leadership and HR. Travel to program sites may be required.

Qualifikationen

• High School Diploma or GED and extensive experience in incident management, investigations, behavioral health, or human services.
• Valid driver's license with a clean driving record.
• Ability to travel to assigned program locations.
• Ability to pass all required background screenings.
• Strong written and verbal communication skills.
• Proficiency with Microsoft Office and electronic documentation systems.

Aufgaben

• Receive, review, and manage reportable incidents across assigned programs.
• Conduct thorough investigations in accordance with organizational policies, state regulations, and federal guidelines.
• Interview staff, individuals served, families, witnesses, and other stakeholders using trauma-informed practices.
• Prepare detailed, objective, and timely investigative reports and supporting documentation.
• Collaborate with program leadership, Human Resources, and other departments regarding investigation outcomes and corrective actions.
• Monitor incident trends, identify root causes, and recommend quality improvement initiatives.
• Participate in incident review committees and follow-up planning activities.
• Assist in developing and monitoring corrective action plans to ensure successful implementation.
• Serve as a subject matter expert on incident reporting systems, documentation standards, and regulatory requirements.
• Provide training and support to staff on incident management processes and best practices.
• Maintain professional relationships with regulatory agencies and external stakeholders.
• Travel to program locations as needed to conduct on-site investigations and reviews.
• Maintain strict confidentiality and uphold the highest ethical standards in all interactions.
