Job summary

RHA Health Services, LLC seeks a Director of Incident and Risk Management to lead enterprise incident response and risk-mitigation across service lines and states. You will oversee intake, triage, investigation, documentation, and resolution of critical incidents to ensure regulatory compliance and policy alignment.

You will build and mentor investigation teams, analyze trends, implement corrective actions, and partner across Operations, HR, Clinical, Legal and Privacy to promote safety,

Qualifications

• Bachelor's degree in a related field.
• Experience leading investigation teams in healthcare or HCBS environments.
• Knowledge of incident reporting standards and HIPAA.

Responsibilities

• Lead enterprise incident-management lifecycle (intake → triage → investigation → actions → closure).
• Oversee investigations, ensure impartial interviews and high-quality reporting.
• Partner with Operations, HR, Clinical, Legal and Privacy to handle sensitive cases.
• Develop training and templates to improve investigation quality and CAPA writing.
