Job summary

Neighborcare Health in Seattle, WA is seeking a Patient Contact Center Representative II - Pacific Tower for on-site work. The role is the first point of contact for patients, providing excellent customer service and facilitating scheduling, care coordination, and outreach in a busy in-person setting.

You will review chart information, navigate the EHR, coordinate with clinical staff, and support patients from diverse backgrounds.

Qualifications

• High School diploma or equivalent required.
• Minimum 2 years in general office or customer service setting.
• Excellent communication and interpersonal abilities.
• Ability to learn processes and software quickly.

Responsibilities

• Conduct chart reviews to extract clinical information for scheduling and care coordination.
• Navigate EHR tabs to verify orders, referrals and appointments.
• Schedule appointments per protocol and availability.
• Act as liaison between external providers and Neighborcare teams.
• Escalate to clinical staff as needed and provide patient support.
• Communicate clearly with patients and staff from diverse backgrounds.
