Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. This PRN position requires providing therapy under supervision, conducting screenings, and supporting evaluation and treatment planning in collaboration with a Physical Therapist.

Responsibilities include individual and group therapy, progress documentation in NetHealth EMR, and coordinating resident schedules with the care team.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and in developing treatment plans with the Physical Therapist.
• Implement individual and group treatments focusing on mobility, strength, balance and ROM.
• Monitor residents' response to therapy and adjust as directed by the Physical Therapist.
• Assist in scheduling and coordinating resident therapies with the team.
• Educate residents, families and staff on therapy protocols and safety.
• Maintain compliant and medically necessary documentation in CMS expectations (NetHealth EMR).

Education
