Job summary

Evolve Therapy Services, LLC is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility in Le Center, MN. Part-time or PRN positions available to provide therapy services, assess residents, and contribute to plan development.

You will document in NetHealth EMR, communicate findings to residents and families, and collaborate with an interdisciplinary team to ensure quality care. Applicants should have a BS in Physical Therapy, an active state license, and ongoing continuing

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop treatment plans with short and long-term goals and monitor progress.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
