Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Le Center, MN. Full-time, part-time, or PRN positions are available to meet resident care needs.

You will plan, organize, and direct physical therapy services, adhering to federal and state standards and CMS guidelines, while collaborating with the interdisciplinary team to ensure high-quality resident outcomes.

Qualifications

• Requires a degree and state licensure.
• Ongoing continuing education to maintain licensure.
• Professional development encouraged.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation and plan implementation.
• Screen residents regularly to determine intervention needs.
• Screen residents referred for therapy to assess need for treatment.
• Evaluate residents to collect data for treatment planning.
• Conduct specialized evaluations as indicated.
• Develop short- and long-term treatment plans and goals.
• Interpret findings and communicate with residents, families, and team.
• Perform other duties to support organization goals.
• Complete CMS-compliant documentation in NetHealth EMR.
