Job summary

Evolve Therapy Services in Saint Paul, MN seeks a compassionate Physical Therapy Assistant (PRN) to support in-house skilled nursing facility residents under the supervision of a Physical Therapist.

Responsibilities include screenings, treatment planning, implementing therapy programs, documenting in NetHealth EMR, educating residents and families, and coordinating schedules with the care team; must hold an active PTA license and an associate degree.

Qualifications

• Associate’s degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and goal setting with the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the care team.
• Train residents, families and appropriate staff in the use or application of equipment.
• Participate in resident, family, and staff education.
• Document compliant and medically necessary therapy notes in NetHealth EMR as required.
