Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Montgomery, Alabama. This PRN position supports ongoing resident care within the therapy department and our partner facility.

Responsibilities include evaluating residents, developing and implementing treatment plans, and ensuring compliant documentation per CMS and company standards. Active licensure and ongoing CEU participation are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention.
• Screen referred residents promptly to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate findings to residents, families, and team.
• Perform other duties to discharge responsibilities in the organization.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR.
