Job summary

Evolve Therapy Services in Birmingham, AL is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role offers Full Time, Part Time, or PRN positions to support resident care and therapy services.

You will assess residents, develop treatment plans, document per CMS expectations in NetHealth EMR, and collaborate with an interdisciplinary team to achieve outcomes.

Candidates should have a BS in Physical Therapy, an active state license, and commitment to ongoing education.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• CMS documentation in NetHealth EMR is required.

Responsibilities

• Provide PT services to residents per standards of practice and department procedures.
• Document and bill therapy services per procedures.
• Evaluate and plan treatment programs with short- and long-term goals.
• Screen residents regularly to determine need for intervention.
• Communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Complete compliant CMS documentation in NetHealth EMR to meet expectations.
