Evolve Therapy Services is seeking a Physical Therapist to provide in-house services at a Skilled Nursing Facility in Birmingham, AL. You will evaluate residents, develop treatment plans, and collaborate with an interdisciplinary team to ensure high-quality resident care.

Requires a BS in Physical Therapy and an active license in the state, with ongoing continuing education. Positions are available as full-time, part-time, or PRN, with competitive pay and benefits.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures.
• Develop treatment plans with short and long-term goals and communicate findings to residents and interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
