Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility located in Selma, AL. The role supports planning, organizing, and directing therapy services to ensure high-quality resident care under CMS guidelines.

Responsibilities include providing PT services, documenting per procedures, conducting screenings, developing treatment plans, and communicating findings to families and the interdisciplinary team. Licensure in the state and ongoing CEUs are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and productivity goals.
• Document and bill physical therapy services per CMS expectations.
• Evaluate residents to inform treatment planning and implementation.
• Develop short- and long-term treatment goals and methods to achieve them.
• Communicate evaluation findings and plans to residents, families, and interdisciplinary team.
